How to Apply

 

1. Apply Online (Fastest)

  • Go to the Ohio Benefits Self-Service Portal
  • Create an OHID account or use your existing one.
  • Set up Multi-Factor Authentication (MFA) for security.
  • Upload your documents through the portal.

 2. Apply by Phone

  • Call 1-844-640-OHIO (6446) and select your language.
  • Virtual hold is available — get a callback without waiting on the line.

 

3. Apply by Paper

  • Fill forms on-site or take them home. Drop them in the secure drop-box — collected daily.
