Ohio Home Care Waiver

The Ohio Home Care Waiver allows those with physical disabilities and unstable medical conditions to receive care in their homes instead of nursing facilities, hospitals or rehabilitation centers. This important service allows these individuals to have more control over their lives and remain active participants in their communities.

Adults with disabilities age 18-59 may be referred to our program by a physician or medical provider. One of our nursing supervisors will meet with them to conduct a needs assessment. If the individual meets the eligibility requirements for the Ohio Home Care Waiver and the financial eligibility for Medicaid, they can be enrolled in the program. One of our skilled case managers will be assigned to help develop a care plan and coordinate services.

How Ohio Home Care Waiver Services are Covered
Service fees are based on eligibility and income. Clients and their families speak with our knowledgeable case managers confidentially about their needs and abilities, and we work with them to develop the best plan for their well-being.

Services offered under the Ohio Home Care Waiver program include:

  • Personal care
  • Skilled nursing
  • Physical/occupational/speech therapy
  • Social work counseling

Individuals are eligible for the Ohio Home Care Waiver if they are:

  • Between 18 and 59 years of age
  • Living in their own home or apartment
  • Requiring hands-on assistance with the activities of daily living
  • In agreement with their physician and provider regarding an approved plan of care
